ALTA LOMA BOARD AND CARE — DSS REQUIRES FACILITY TO FILE A LIC 500 REPORT
Without sufficient staffing, elderly residents in a RCFE (Assisted Living Facility, board and care home, etc.) may not get the care and assistance they require for their overall health and safety. This is especially true with elderly residents suffering who require assistance with their activities of daily living. To help ensure RCFEs in California are sufficiently staffed, the State of California requires all RCFEs to file LIC500 reports with the DSS. This report identifies all facility personnel and all other adults living and working at the facility. By reviewing the report, DSS investigators determine if the owners/managers of the RCFE have adequately staffed the facility so the residents living in the facility can receive the care, supervision and assistance they require. Alta Loma failed to have a LIC500 on file and was required to do so following the last DSS investigation of the facility. Alta Loma Board and Care in Rancho Cucamonga
About the Facility: Alta Loma Board and Care is a 6-bed board and care facility (aka RCFE) located on 6368 Moonstone Ave., Rancho Cucamonga, CA 91701. The facility is owned/managed by Alta Loma Board and Care, Inc. As of January 2018, DSS records identified Gina Cayanan and F. Cayanan as the Administrators of the facility. The facility is cleared for six (6) non-ambulatory residents one of which may be bedridden. The facility is also approved for Dementia Services.
DSS Records: According to DSS public records, following its annual investigation visit of 2018, Alta Loma Board and Care was not cited for any deficiencies. The facility was required to remove a hose left out on the backyard sidewalk which was considered an obstruction and required to remove a Clorox container left on the patio which a dementia resident could get access to. The facility was also required to file a LIC 308 – Designation of Facility Responsibility, a LIC 610E – Emergency Disaster Plan for Residential Care for the Elderly and a LIC 500: Personnel Report.
Overall, it appears this facility received a good report from the DSS in 2018. Nevertheless, if your loved-one is a resident of Alta Loma Board and Care and you have concerns about the care they are receiving, don’t ignore your concerns. Consider making an unnanounced visit to the facility when they are not expecting you to show up. Many times, unexpected and unannounced visits will reveal areas of inadequate care or services that need to be addressed. It is also important to keep a lookout for common signs of elder abuse and elder neglect.

When a resident of a board and care home (aka RCFE) violates state law and causes an injury to a patient, or the resident endures acts of elder abuse or elder neglect which cause physical, mental and/or emotional injury, the board and care facility should be held accountable for its actions. A victim or his/her family can sue for:
- the elderly victim’s pain and suffering;
- the victim’s wrongful death if the victim of elder abuse or elder neglect died as a result of the elder abuse or elder neglect;
- negligent infliction of emotional distress suffered by close family members who observed the elder abuse or elder neglect.
As provided in Welfare & Institutions Code § 15657 in cases of reckless, oppressive, malicious and/or fraudulent elder abuse and/or elder neglect, the elderly victim or his family is entitled to recover enhanced remedies under the Elder Abuse/Neglect Act, and may also seek punitive damages to discourage future acts of elder abuse and elder neglect in California nursing homes.
