ALTA LOMA BOARD AND CARE — DSS REQUIRES FACILITY TO FILE A LIC 500 REPORT
Without sufficient staffing, elderly residents in a RCFE (Assisted Living Facility, board and care home, etc.) may not get the care and assistance they require for their overall health and safety. This is especially true with elderly residents suffering who require assistance with their activities of daily living. To help ensure RCFEs in California are sufficiently staffed, the State of California requires all RCFEs to file LIC500 reports with the DSS. This report identifies all facility personnel and all other adults living and working at the facility. By reviewing the report, DSS investigators determine if the owners/managers of the RCFE have adequately staffed the facility so the residents living in the facility can receive the care, supervision and assistance they require. Alta Loma failed to have a LIC500 on file and was required to do so following the last DSS investigation of the facility. See below for more information on Alta Loma Board and Care in Rancho Cucamonga.
