COULD PROVIDENCE ONTARIO’S ABYSMAL MEDICARE OVERALL RATING OF TWO OUT OF FIVE STARS BE THE REASON FOR ITS RECENT NAME CHANGE TO LAS COLINAS POST ACUTE?
Medicare provides a star rating system for nursing homes throughout the United States. Providence Ontario is a nursing home owned/managed by Ontarioidence opco, LLC since about 2015. As last reported by Medicare, the facility had an abysmal overall rating of two out of five stars with one out of five stars in Health Inspections rating and staffing rating. SNF records kept by the CDPH now report that the name of the facility has been changed to Las Colinas Post Acute. The name of the licensee (Ontarioidence Opco, LLC) remains the same.
Time will tell whether the name change will result in needed improvement in the overall rating of the care and services provided by the facility. See below for more information on Las Colinas Post Acute.
