Las Colinas Post Acute fka Providence Ontario (216-bed SNF)

COULD PROVIDENCE ONTARIO’S ABYSMAL MEDICARE OVERALL RATING OF TWO OUT OF FIVE STARS BE THE REASON FOR ITS RECENT NAME CHANGE TO LAS COLINAS POST ACUTE?

Medicare provides a star rating system for nursing homes throughout the United States. Providence Ontario is a nursing home owned/managed by Ontarioidence opco, LLC since about 2015. As last reported by Medicare, the facility had an abysmal overall rating of two out of five stars with one out of five stars in Health Inspections rating and staffing rating. SNF records kept by the CDPH now report that the name of the facility has been changed to Las Colinas Post Acute. The name of the licensee (Ontarioidence Opco, LLC) remains the same.

Time will tell whether the name change will result in needed improvement in the overall rating of the care and services provided by the facility. See below for more information on Las Colinas Post Acute.

About the Facility: Las Colinas Post Acute is a 216-bed for-profit nursing home (skilled nursing facility aka SNF) located at 800 East Fifth St., Ontario, CA, 91764 which is owned/managed by Ontarioidence opco, LLC.

Medicare’s Overall Quality Rating: Medicare’s overall quality rating of Providence Ontario (now known as Las Colinas Post Acute) was most recently two stars (out of a possible 5 stars) which is a BELOW AVERAGE rating.

Staff-to-Patient Ratios: The Long Term Care Community Coalition (LTCCC) provides staffing data for nursing homes throughout the country. However, as of September 18, 2018, no data was available for either Providence Ontario or Las Colinas Post Acute.

CDPH Records: Investigative surveys conducted by the CDPH in 2018 resulted in Providence Ontario (now known as Las Colinas Post Acute) being served with notices of DEFICIENCIES for violation of federal and/or state law, including the following:

• Violation of Title 22 72301(f): Failing to keep the indwelling catheter of a patient in a position where it would drain urine properly having the potential to cause the patient pain, bloating and infection. The CNA on duty said she did not get a chance to put position the catheter properly.
• Violation of Title 22 72631(b): Failing to provide a patient with easy access to a call light leaving the patient without the ability to call for held or assistance if needed. The call light was found on the floor on the side of the bed away from where the patient was sitting.
• Violation of Title 22 72567(a): The facility’s emergency medical cart was missing normal saline for use in a medical emergency.
